Employers earn rewards, such as premium renewals discounts, when they successfully complete Health Academy’s annual wellness activities at specific participation levels.

Annual Health Management Activities
Employers who join Chamber Health Plus must complete these health management activities annually:
1. Enroll in Health Academy
2. Offer an annual health day for employees. Many employers will opt to hold their biometric screenings on the annual health day.
3. Provide a Health Risk Assessment questionnaire for employees, through Health Academy
4. Complete an annual on-site biometric screening for employees, through Health Academy
5. Establish one of three responsible worksite policies

- A dedicated health coordinator who facilitates health risk assessments and annual biometric screenings
- Health Risk Assessment questionnaire for employees and spouses

- Reporting, including confidential individual reports for employees and group reports for employers
The Employer, the Select Broker and Health Academy
A successful health management program requires dedication, commitment and expertise. Employers and their Select Brokers are responsible for program execution and meeting Chamber Health Plus membership requirements.
